Thousands of trivial clinical entries upgraded to Major Problem Empty Thousands of trivial clinical entries upgraded to Major Problem

Since about the middle of last week we've been noticing that thousands of trivial clinical entries have been upgraded to a Major Problem. Obviously this is messing up our Clinical Tree quite significantly.

Yesterday 22/5/14, TPP admitted it was a cock-up on their part:


Problems
In the last Maintenance Release, TPP implemented a change that caused all Summary items recorded to be duplicated as Problems. This change was made in preparation for the deployment of new Problems functionality.

Following user feedback, TPP have now reverted this change.

How widespread is this problem? We are on SystmOne Prison so we initially thought it was just a problem with our version but the comments would seem to suggest it is a more widespread thing.

So far no sign of a remediation plan from SystmOne or even an apology. I don't think they quite realise how their unthinking act has vandalised the Problem Oriented medical records of thousands, possibly MILLIONS of patients!

In the last Maintenance Release, TPP implemented a change that caused all Summary items recorded to be duplicated as Problems. This change was made in preparation for the deployment of new Problems functionality.

Following user feedback, TPP reverted this change last Thursday. Since then, TPP have been removing the additional problem headers that were recorded in error. As long as that problem header has not been used for further action, the duplications will now be permanently removed from the record.

Records for patients who are part of the new Problems functionality pilot will not be amended by TPP.  

TPP apologise for any inconvenience that this maintenance change has caused.
